The Trust employs full time nurses who assist in the medical management of complex cases. The goal of the Nurse Case Manager is to coordinate services aimed at ensuring quality medical care while containing claim costs, in an effort to assist the injured worker in reaching maximum medical improvement and returning to gainful employment.
The Nurse Case Manager coordinates medical and rehabilitation services for injured workers receiving workers' compensation benefits.
The Nurse Case Manager will assess medical, psychological, rehabilitation and vocational needs of these individuals.
Cases referred to the Nurse Case Manager have phone contact within 24 hours to the injured worker to schedule an initial visit.
During the visit, the following information is gathered:
Contact is then made with the treating provider and employer, and a treatment plan is established.
The Nurse Case Manager works closely with the claims adjusters to keep them informed and updated on the medical treatment of the injured worker.
